Enrollment Documents
The following documents are required to be on file for a child to attend our program.
Please fill out and email completed forms to hadassahbrofman@gmail.com or return to our location.
These must be received no later than August 1st of each school year.
Or for families who would like a vaccine exemption, you must visit: https://cdphe.colorado.gov/exemptions-to-school-required-vaccines
and complete the course to receive the exemption certificate.
